Tooth #31 with “nightmare” access: minimal vertical clearance and deep site, featuring ThinLayer™ GuideThis case involves “nightmare access” to site #31. The patient had limited mouth opening, the molar site was deep and positioned quite posterior, and there was bone loss on the distal of tooth #30. A traditional “tube” guide could not be used due to vertical space constraints, so a ThinLayer™ Guide from Guided Surgery Solutions. These guides require no more vertical space than freehand drilling. The set of ThinLayer™ guides included an on-surface guide and an off-surface guide. The on-surface guide contains a guide hole on the gingival surface and was used to create an initial pilot hole. The guide hole at the occlusal surface in the off-surface guide is identical to the top of a guide tube in a tubed guide. The pilot hole and off-surface guide form two points that – in sequence – define a trajectory, without the need for a tube. After the pilot hole has been prepared, the on-surface guide is removed and the off-surface ThinLayer™ guide is placed. The guide allows the flaps to be reflected with visual and physical access to the surgical site. The surgical plan was developed in Blue Sky Plan, with cross-sectional, axial, panoramic, and 3D views of the bone, gingiva, and teeth.
